FY 2009 Airport State Funding Application

Completed applications are due in the Office of Aviation by 4:00 p.m. on Friday, May 2, 2008.  No late applications will be considered.  Applications may be e-mailed if they include sponsor signatures.

Complete applications will include:

•       Airport Funding Grant Application – Form 291114 (also includes an application checklist)
   

•       Airport Funding Application Project Data Sheet – Form 291115 (complete one for each
  project
   

•       5-Year Capital Improvement Program (CIP) – Form 291112
   

•       Resolution from local governmental jurisdiction endorsing the project
   

•       Planning documents as required (project shown on ALP, security plan, pavement management plan)

Applications can be mailed to:  Office of Aviation, Iowa DOT, 800 Lincoln Way, Ames, IA 50010, or e-mailed to kay.thede@dot.iowa.gov.
